Master Yang’s Martial Arts
Participant Agreement, Liability Waiver, and School PoliciesI, the undersigned participant, or the parent/guardian of the participant (if under 18 years of age), acknowledge and agree to the following:Assumption of RiskI understand and acknowledge that participation in martial arts training, including Taekwondo, involves inherent risks of injury or harm. These may include physical injury, psychological stress, pain, illness, temporary or permanent disability, or death. Such risks may result from my own actions or negligence, the actions or negligence of others, the condition of the premises or equipment, or the nature of martial arts activities themselves.
I voluntarily assume full responsibility for all risks associated with participation in martial arts training and related activities.Release and Waiver of LiabilityI, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release and discharge Master Yang’s Martial Arts, its owners, directors, officers, employees, instructors, volunteers, agents, and representatives (“Released Parties”) from all claims, liabilities, or causes of action arising from or related to my participation in martial arts training and related activities.
This waiver includes claims resulting from negligence but does not apply to cases of gross negligence or intentional misconduct.
This release is intended to be as broad and inclusive as permitted under Idaho law. If any portion is deemed invalid, the remainder shall remain enforceable.IndemnificationI agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, expenses, and attorney’s fees arising from my participation in martial arts activities, whether caused by negligence or otherwise, except in cases of gross negligence or intentional misconduct.Medical TreatmentI authorize Master Yang’s Martial Arts to obtain emergency medical treatment for me (or my child) if necessary. I understand I am responsible for all costs of such treatment, including transportation, hospitalization, and medications.
I release the Released Parties from any liability related to first aid or medical care rendered during participation.Parental Acknowledgement (For Participants Under 18)As the parent/guardian, I acknowledge the inherent risks of martial arts training and voluntarily assume them on behalf of the minor participant. I agree to release and hold harmless the Released Parties from liability for any injury or damage, except in cases of gross negligence or willful misconduct.Photographic and Media ReleaseI grant Master Yang’s Martial Arts permission to use photographs, videos, or other media depicting me or my child for promotional, educational, or marketing purposes without compensation.
I waive any right to royalties or other compensation related to the use of such materials.Compliance with Rules and RegulationsI agree to comply with all school rules, regulations, and instructor directions. Failure to do so may result in suspension or dismissal from the program without refund.
Master Yang’s Martial Arts reserves the right to deny or revoke participation to any individual deemed unfit or noncompliant.No GuaranteesI understand that martial arts progress and belt promotion depend on the participant’s individual effort, attendance, and discipline. Master Yang’s Martial Arts makes no guarantees of specific results or outcomes.No Compensation for InjuriesI understand that Master Yang’s Martial Arts is not responsible for covering medical bills, lost wages, or other financial losses arising from injury, illness, or harm sustained during participation, except for claims involving gross negligence or intentional misconduct.SeverabilityIf any portion of this agreement is found unenforceable, all remaining provisions shall continue in full force and effect.Cancellation and Refund PolicyAt Master Yang’s Martial Arts, we value transparency and fairness. Please review the following terms carefully before enrolling:Contract BindingOnce signed, enrollment contracts are legally binding under Idaho law. Students are encouraged to fully review all terms prior to signing. Early termination may result in financial obligations.Cancellation Policy30-Day Written Notice is required to cancel enrollment. Notices must be submitted by email or postal mail.Early Termination Fee: If a contract is ended before its term, the student must pay an early termination fee equal to 25% of the remaining balance.No Refund PolicyAll Fees Non-Refundable: Enrollment, administrative, and service fees are non-refundable, including cases of early cancellation or missed classes.Prorated Refunds: Prepaid services not yet provided may qualify for a prorated refund minus the 25% early termination fee.Refund Timeline: Eligible refunds will be processed within 30 days using the original payment method.No Refunds for Completed Services: Services already rendered or classes attended are non-refundable.Extensions and SuspensionsMembership Extension: In cases of medical issues, relocation, or valid hardship, a membership term may be extended. Payments must continue during the term, and the missed period may be added at the end.No Suspensions: Memberships cannot be paused or suspended; payments continue as scheduled even during absences.Compliance with Idaho LawAll policies adhere to Idaho state laws on contracts and consumer rights. These terms ensure fairness and protection for both the school and its students.Policies and ProceduresEnrollment and MembershipAll students must complete enrollment and sign the liability waiver before participating.
Membership fees begin on the enrollment date.
Consistent attendance is essential for progress and promotion.Attendance PolicyArrive 10 minutes before class begins.
Notify the school by phone or email at least 24 hours in advance if unable to attend.
Excessive unexcused absences may delay belt progression.Behavior and DisciplineShow respect toward instructors, peers, and the Dojang at all times.
Inappropriate behavior may result in a warning or suspension.
Proper uniform and safety gear must be worn for every class.Safety ProceduresFollow all instructor directions and safety rules.
Report any injuries or unsafe conditions immediately to an instructor.Medical EmergenciesIn case of a severe injury or medical emergency, staff will call 911 and provide appropriate assistance.
Emergency contact and medical info for all students are kept on file for quick access.Fire and EvacuationStudents are familiarized with exit routes each term.
Fire drills are conducted regularly.
In case of fire, evacuate calmly and do not re-enter until cleared by authorities.Severe WeatherThe school monitors local weather alerts.
Classes may be canceled or rescheduled in severe conditions.
Notifications will be sent via email and posted on social media.Threats or ViolenceIn the event of a threat, doors will be locked and students moved to a secure area.
Law enforcement will be contacted if needed.
